Original and copy fo the Birth Certificate or Register of the Abroad Born Peruvian from the Peruvian father or mother that request the registration and a copy of the Identity Document. |

Original and copy of the identity document that entered to the country and a copy of the Andean Immigration Card (TAM). |

Two (2) pages of filiation of the minor signed by the father and mother or guardian duly identified (with Power of Attorney) and endorsed by the Chief of Immigration if the procedure begins in provinces. (ONLY FOR PROVINCES). |
