Receipt of payment - Banco de la Nación for the right of procedure (S/. 21.00).

A copy of the valid Alien Registration Card (Annual Rate of Immigration and Annual Rate of Residence Extension).

Original and copy of the passport.

Letter of Guarantee legalized by the Public Notary or Certificate of Tax assessment issued by SUNAT.
06
A copy of the air ticket.
07
In case of theft or lost submit the ORIGINAL ALIEN CARD or REPORT issued by the Police Station.
Cancellation of Residence (If your last entry was as a TOURIST)

Form F-007.

Receipt of payment - Banco de la Nación (S/. 21.60).

A copy of the Alien Registration Card (Annual Rate of Immigration and Annual Rate of Residence Extension).

A copy of the passport.

Letter of Guarantee legalized by a Public Notary.

In case of theft or lost submit the ORIGINAL CARD or REPORT issued by the Police Station.

The Alien Registration Card should be valid in respect to the last exit as a resident.

A copy of the identification card of the relative who will deliver the original card.

Qualification: Required Evaluation.
Procedure Timeline: Eight (8) days.
Department in Which Starts the Procedure: DIGEMIN or Immigration Headquartes.
  Authority Who Approves the Procedure: Director de Inmigraciones or Sub-directores or Jefes de Migraciones.
  Authority in Case of Reconcideration:Director de Inmigraciones or Sub-direcciones or Jefes de Migraciones.
Authority In Case Of Appeal: General Director of Immigration and Naturalization.
